Get started with your Medicare Advantage enrollment by following these three easy steps

1. Make Sure You’re Eligible

You can enroll in a UCLA Health Medicare Advantage HMO Plan if you answer "yes" to these two questions:

  1. Do you have Medicare Part A and Part B? If not, contact Social Security.
  2. Do you live in Los Angeles County?

2. Check Your Enrollment Period Qualification

You can enroll during certain times of the year or after specific life events, as detailed below:

  • The Initial Medicare Advantage Enrollment Period is a 7-month window that begins three months before your 65th birthday and ends three months after your birthday month. If you’re less than 65 and entitled to Medicare based on disability, you can enroll three months before and after your 25th month of disability benefit entitlement.
  • The Medicare Annual Enrollment Period occurs every year from October 15 to December 7. You can sign up for, switch, or leave a Medicare Advantage plan during this time.
  • The Medicare Advantage Open Enrollment Period occurs every year from January 1 to March 31. During this period, you can switch between Medicare Advantage plans or back to Basic Medicare if you’re unsatisfied with your Annual Enrollment Period plan selection.
  • The Special Enrollment Period is triggered by certain qualifying events, including losing employer coverage, retiring, moving, and other situations. Enrollment timing depends on the situation.

3. Enroll in a UCLA Health Medicare Advantage Plan

Review and choose our UCLA Health Medicare Advantage Principal or Prestige Plan. Then, apply by phone, mail, email, fax, or at an upcoming seminar. Be sure to have your Medicare ID card handy.

What to Expect After You Enroll

After completing your Medicare Advantage enrollment, you’ll receive a confirmation letter and call from Member Services to welcome you to the plan and answer any initial questions. You may receive an additional letter if you enroll with a licensed agent.

Your UCLA Health Medicare Advantage Plan Member Welcome Kit and member ID card will arrive in the mail about 10 days after your application is processed. Be sure to bring your card when you visit your doctor, pharmacy, or dentist. You can also access your member ID on the MyChart Member Portal.

Lastly, you’ll receive your Smart Benefits Card with instructions on activating and using it. Your card will arrive separately from your welcome kit.
